I understand and agree to the following in becoming a member of NZMPS:

1. I appoint NZ Unions as my agent to act for me with respect to the relevant union, employee and/or professional association nominated by me, including, if requested and authorised by me, to advise my resignation from any other professional association and/or union of which I may currently be a member.

2. Such an appointment is to apply until withdrawn by me in writing.

4. I confirm that I am joining a member-based professional association that provides members various services on a not-for-profit basis.

5. I authorise NZ Unions to establish an automatically renewing subscription payment at the interval selected by me during sign up.

6. I agree that NZMPS and its agents may share basic contact and employment information with my employer where needed to support or represent me at work.

7.   I understand and consent to receiving SMS and emails relating to my membership, subscriptions, and services.

8. I declare the details provided in this application are, to the best of my knowledge, true and accurate.
