1. I appoint NZ Unions as my agent to act for me with respect to the relevant union, employee and/or professional association nominated by me, including, if requested and authorised by me, to advise my resignation from any other professional association and/or union of which I may currently be a member.
2. I authorise NZ Unions to apply on my behalf for membership of the association known as the New Zealand Medical Professionals' Society (NZMPS) and agree to abide by its Constitution, by-laws and policies and procedures. I understand my membership application is subject to the terms as set out in this application.
3. I appoint a third-party nominated representative of my choice mutually agreed to by NZMPS, or a representative of the association known as NZMPS, NZMPS itself, or jointly as above with NZ Unions or its nominees, to act as my bargaining agent or representative, and authorise them to represent me in any industrial matter or dispute. Such an appointment is to apply until withdrawn by me in writing.
4. I confirm that I am joining a member-based professional association/ union that provides members various services on a not-for-profit basis.
5. I authorise NZ Unions to establish an automatically renewing subscription payment at the interval selected by me during sign up.
6. I declare the details provided in this application are, to the best of my knowledge, true and accurate.
